The Nutritional Supplement Industry: A Poor Reputation
- Nutritional supplements and prescription drugs are not regulated at the same standards by the FDA.
The Dietary Supplement Health and Education Act (DSHEA) legislates that the FDA has minimal authority in regulating the nutritional supplement industry. Hence nutritional supplements are not tested for safety before they are put onto the market. Only recently (June 22, 2007) has the FDA mandated that products actually contain what is written in the supplements facts on the bottle. After a product has been on the market, the FDA has more authority to evaluate the safety and contents of a product. Because of this relatively low level of oversight, the supplement industry has become confusing with a disproportionate level of marketing. There are literally thousands of products for sale. Marketers use the internet, phone, email, direct mail, traditional retail, health stores, television and radio to promote their products. Consumers are presented with a dizzying array of potential products and there is little education provided by the companies themselves. - Effectiveness.
Supplement manufacturers are generally prohibited from making any claims about the efficacy of their products. Through careful wording, many manufacturers have been able to sell questionable products to hopeful patients without any clinical or scientific support for their product's contents. Despite all of the noise in the industry, there are good clinical trials that have taken place that validate the efficacy of certain compounds, vitamins and extracts in certain concentrations, with certain excipients, when given to a specific patient profile. It is important that these factors are considered in the context of a particular patient’s disease and other conditions. - Accuracy and Purity.
It is currently unclear whether or not the FDA has authority to inspect every aspect of a supplement manufacturer. Unlike the stringent reviews and inspections for the pharmaceutical industry, there have historically been no reviews of the supplement industry. As result of the lack of verification and review, manufactures are left to their own devices as to whether or not they will include all of the ingredients in the amounts advertised on their labels. Over the years, there have been numerous reports numerous reports of product that fail to contain the amount of ingredients they claim on their label. It is expected that the June, 2007 announcement by the DSHEA will change this, but it remains to be seen how this new regulation will be applied to industry. - Medication Interactions.
Even natural or herbal substances can be unsafe in the wrong quantities, or more commonly, when consumed in conjunction with medications or other food items. It is important that a physician be allowed to consider all of the elements of nutrition when assessing one's health. For example, some elements of garlic extracts (ajoene for example) can thin the blood. It is not recommended that a patient on coumadin take garlic as they will get blood thinning beyond the amount the physician targeted in making the prescription.
